Children:
Who need skilled pediatric nursing care
Adults:
18 years old and up
Disability:
Developmental disability (DD) with an IQ of 70 or less and/or dual diagnosis of developmental disability and mental illness(DD/MI)
Eligibility:
Medicaid eligible
Residence:
Must reside in the counties of Kane and/or Kendall, or in Hanover Township of Cook County or have meaningful ties to the service area
