SERVICES PROVIDED

• Assistance with accessing services to meet the individual’s needs
• Act as advocates for the individual to ensure that the service plan is consistent with the strengths, needs and choices of the individual
• Implementation of the service plan is followed up regularly by visits with the individual at both their home and day program
