SERVICES PROVIDED

Your family is entitled to (at no cost):

  • Child and Family Connections Staff
  • Service Coordinators
  • Do intake visits to explain the program and collect basic information
  • They will assure evaluations (including Medical diagnostic evaluations), assessments, and Individualized Family Service Plans (IFSP) are completed to meet your needs
  • They will inform you of your rights under the Early Intervention System, and link you to the services you choose to meet your child and family’s needs
  • If desired, the Service Coordinator can assist the family in transitioning to the school district special education preschool to continue services after age three
  • Parent liaison can link you to a family with common concerns and experiences, offer additional support, information and resources, and assure services provided are family centered. The Parent liaison is a parent who has a child with developmental delays/disabilities and can offer assistance on an informal and personal basis
  • Local Interagency Council Coordinator (LIC) is housed at Child and Family Connections. The LIC is a group of concerned parents and providers who plan, implement, and evaluate the Early Intervention service system in Kane and Kendall counties. The council is one way for the parents to have input into the services that are available in their communities for families and children.
  • Evaluations in any developmental areas of concern to determine eligibility for the program.

After eligibility is determined:

  • Evaluations are still free
  • A monthly family fee is assessed. The fee is based on income and family size.
  • Additional services paid for by either family insurance and/or the State of Illinois, may be provided by professionals trained in one or more of the following areas:
    • Audiology
    • Aural Rehabilitation
    • Assistive Technology
    • Developmental Therapy
    • Family Training and Support
    • Health Services
    • Medical Services
    • Nursing Services
    • Nutrition Services
    • Occupational Services
    • Physical Therapy
    • Psychological Services
    • Social Work Services
    • Speech Language Therapy
    • Transportation
    • Vision Services
