Individuals with a diagnosis of developmental
disabilities (DD) or who are dually
diagnosed with Mental Illness and Developmental
Disabilities(DD/MI).
Residing in or had resided in a Nursing
Home, in the state of Illinois for 120
days in accordance with the Nursing
Home Reform Act, between 1986 and April
1, 1994.
