BRIEF HISTORY

The Bogard Service Coordination program serves individuals who are part of a class action lawsuit brought against the State of Illinois in 1986. The lawsuit stated that the active treatment needs of individuals with developmental disabilities or with dual diagnosis of mental illness and developmental disabilities, living in Nursing Homes, were not being met in accordance with the Nursing Home Reform Act. This resulted in a Consent Decree in the fall of 1993, that mandated the Department of Human Services (DHS), through Pre-Admission Screening and Service Coordination agencies, provide service coordination and advocacy to the members of this class action lawsuit.
