DayOneNetwork was incorporated in 1989 through the effort of community leaders in Kane and Kendall Counties and Hanover Township of Cook County. DayOneNetwork was founded in order to facilitate and better coordinate a wide variety of services to individuals who have disabilities.
DayOneNetwork is a private, nonprofit, community based agency providing service coordination and advocacy services to children and adults with developmental delays and/or disabilities of all ages. The agency "guides individuals and their families through the maze of community services to access needed supports and services". Our ultimate goal is to connect people who need services with those individuals or agencies that provide them.
The mission of DayOne Network is to find solutions for children and adults with developmental delays and/or disabilities in Kane and Kendall Counties and Hanover Township of Cook County and to make the connections for necessary services and supports.
This mission is accomplished through independent service coordination activities:
- Screening
- Eligibility Determination
- Assessment and Evaluation
- Service Plan Development
- Service Arrangement and Linkage
- Follow Up
- Advocacy and Community Education
- Information and Referral
The agency currently provides:
Child and Family Connections
Support for families with infants or toddlers ages birth to 36 months who have concerns about their child's development.
Community Access
Pre-Admission screening to determine eligibility
Bogard Individual Service Coordination
To ensure active treatment for seniors in nursing facilities.
Service Coordination
For individuals of all ages with developmental disabilities, including:
- Providing information and referrals
- Supporting Community Awareness and Education
Individual Services and Support Advocacy
- Assistance with accessing services to meet the individual's needs
- Act as advocates for the individual to ensure that the service plan is consistent with the strengths, needs and choices of the individual
- Implementation of the service plan is followed up regularly by visits with the individual at both their home and day program
BOARD OF DIRECTORS
DayOneNetwork has a voluntary Board of
Directors that consists of not more than
15 board members which meet the 3rd Wednesday
of the month, every other month. The Board
of Directors are responsible for overseeing
the long term planning for DayOne as well
as setting new policy guidelines and assisting
in the various fundraising activities.
In addition, each Board Member is required
to serve on at least one Committee and
attend the Committee meetings.
